ELEVATION accepts several forms of insurance and we are in the process of growing our provider list. We will call to verify your benefits, but ultimately, each patient is responsible to ensure coverage. Colorado is a direct access state, meaning you do not need a referral to come see us, HOWEVER, some insurance programs still require a doctors referral. Please check with your provider to verify your requirements to seek out service.

In order to get the most time out of your visit, please fill out the following forms prior to your first visit and bring them with you. ELEVATION offers the most competitive cash-pay rates for services in the Denver Metro Area. If you elect not to go through an insurance provider, we have a variety of programs and packages to help you recover without breaking the bank. A vast majority of our patients elect to save some paperwork and go this route. We have a healthy referral discount program available and offer service discounts for our business partner members.
